Part II: HIPAA Notice of Privacy Practices (For Patients)
THIS SECTION DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This section applies specifically to the Protected Health Information (PHI) of our patients. The Limitation of Liability described in Section I.D. does not apply to your rights regarding your PHI under HIPAA.
A. Our Legal Duty
We are required by law to maintain the privacy of your PHI, provide you with this Notice, and notify you following a breach of unsecured PHI.

We engage reputable, HIPAA-compliant third-party vendors for communications and data management. For any vendor that handles PHI, we execute a formal Business Associate Agreement (BAA), legally requiring them to protect your information.
