The following Terms and Conditions are designed to ensure that we can serve all our patients effectively, maintain a safe clinical environment, and allocate the dedicated time and resources necessary for your treatment. Your commitment to your appointments and our administrative requirements is critical to the success of your care.
This policy outlines our mutual commitments regarding appointment scheduling, mandatory pre-treatment compliance, financial responsibility, and patient conduct.
Article I: Appointment Deposits, Cancellations, Late Arrivals, and No-Shows To secure an appointment, a non-refundable deposit is required at the time of booking. This deposit reserves the provider’s dedicated time, the treatment room, and any necessary specialized equipment for your exclusive use.
Rescheduling: We understand that scheduling adjustments are sometimes necessary. Should you need to reschedule your appointment, we require a minimum of 48 business hours’ notice. With such notice, your deposit will be fully applied to your rescheduled appointment.
Late Cancellation & No-Show Financial Policy: Our practice strictly enforces its cancellation policy. If an appointment is cancelled or rescheduled with less than 48 business hours’ notice, or in the event of a “no-show,” the patient forfeits their deposit and will be charged the full, non-discounted price of the scheduled treatment. The remaining balance for the missed service will be charged to the credit card on file.
Late Arrivals: Time is precisely allocated for your care. If you arrive late, we may not be able to safely or adequately accommodate your appointment. Late arrivals that cannot be accommodated will be classified as a late cancellation, resulting in the forfeiture of your treatment, your deposit, and the application of the full scheduled treatment charge.
Policy for Patients with Treatment Series: For patients who have pre-paid for a treatment series or package, a late cancellation, no-show, or unaccommodated late arrival will result in the immediate forfeiture of one full session from their existing package.
Article II: Mandatory Pre-Appointment Compliance and Identity Verification For your medical safety and our legal compliance, specific administrative steps must be finalized before any treatment can commence.
Paperwork and ID Verification: All patient intake forms, medical histories, requested document uploads, and identity verification procedures must be completed in their entirety prior to your scheduled appointment.
Forfeiture for Non-Compliance: Failure to complete all required paperwork, upload requested documents, or successfully verify your identity prior to your appointment time will result in the immediate cancellation of your appointment. Under these circumstances, you forfeit your scheduled treatment, your deposit, and any right to a refund.
Article III: Code of Conduct and Right to Refuse Service
Right to Refuse Service: Clinical Care 365 reserves the right to refuse service to anyone at our sole discretion, at any time, for any reason, to the maximum extent permitted by law.
Zero Tolerance Policy: We maintain a strict zero-tolerance policy for poor, disruptive, or abusive behavior toward our staff, providers, or other patients. Failure to follow clinic policies or exhibiting inappropriate conduct will result in the immediate termination of the provider-patient relationship, dismissal from the premises, and the forfeiture of any pending treatments, packages, and associated payments or deposits without refund.
Article IV: Commitment to Treatment Series and Financial Responsibility Our practice offers treatment series or packages at a preferential rate. This discounted pricing is offered based on the patient’s commitment to completing the entire series as prescribed by their provider, allowing us to achieve optimal clinical outcomes.
Non-Refundable and Non-Transferable: When you commit to and purchase a treatment series, you are securing a block of provider time and resources at a discounted value. As such, payments made for treatment series are strictly non-refundable and non-transferable to other individuals.
Financial Responsibility: By purchasing a treatment series, the patient acknowledges and agrees that they are responsible for the full financial commitment of the package. Incomplete sessions from a purchased series will not be refunded. Discontinuing a treatment series does not release the patient from their financial responsibility for the entire purchased package.
Valuation of Rendered Services: If a patient is unable to complete a purchased series for any reason, any treatments that have already been rendered will be retroactively valued at their full, non-discounted, individual session price.
Article V: Financial Disputes and Chargeback Waiver
Chargeback Waiver: By providing a credit card on file, paying a deposit, or purchasing a treatment or package, the patient unequivocally agrees to the terms outlined in this policy. The patient expressly waives the right to initiate a credit card chargeback, dispute, or claim with their bank or credit card provider for any fees legally levied under this policy, including but not limited to non-refundable deposits, no-show fees, late cancellation fees, and pre-paid non-refundable treatment packages.
Authorization: The patient unequivocally authorizes the practice to charge the card on file for any and all balances owed under these Terms and Conditions.
Article VI: Binding Agreement and Patient Acknowledgment This policy is publicly posted on our official company website and is accessible to all patients prior to booking. The following terms constitute a legally binding agreement.
Agreement by Performance: By scheduling an appointment, tendering a deposit, or providing payment information, the patient acknowledges that they have had the opportunity to review these terms and agrees to be fully bound by them. The act of booking an appointment serves as the patient’s express and legally binding consent to this entire policy.
Policy Supersedes Incomplete Paperwork: This agreement is fully enforceable irrespective of whether the patient has completed, signed, or submitted their formal intake paperwork, consent forms, or ID verification. The financial commitment and adherence to these Terms and Conditions are established at the exact time of booking and are not contingent upon the subsequent completion of administrative forms.
