The following policy is designed to ensure that we can serve all our patients effectively and provide the dedicated time and resources necessary for your treatment. Your commitment to your appointments is critical to the success of your treatment plan. This policy outlines our mutual commitments regarding appointment scheduling, deposits, and pre-paid treatment series.

Article I: Appointment Deposits, Cancellation, and No-Show Policy

To secure an appointment, a non-refundable deposit is required at the time of booking. This deposit reserves the provider’s dedicated time, the treatment room, and any necessary specialized equipment for your exclusive use.

  • Rescheduling: We understand that scheduling adjustments are sometimes necessary. Should you need to reschedule your appointment, we require a minimum of 48 business hours’ notice. With such notice, your deposit will be fully applied to your rescheduled appointment.
  • Late Cancellation & No-Show Financial Policy: Our practice has a strict cancellation policy. If an appointment is cancelled or rescheduled with less than 48 business hours’ notice, or in the event of a “no-show,” the patient will be charged the full, non-discounted price of the scheduled treatment.
    • Any deposit on file for the appointment will be applied toward this charge.
    • The remaining balance for the missed service will be charged to the credit card on file.
    • By providing payment information to secure an appointment, the patient unequivocally agrees to and authorizes the practice to levy this charge in accordance with the terms of this policy.
  • Policy for Patients with Treatment Series: For patients who have pre-paid for a treatment series or package, a late cancellation or no-show will result in the forfeiture of one full session from their existing package.

Article II: Commitment to Treatment Series and Financial Responsibility

Our practice offers treatment series or packages at a preferential rate. This discounted pricing is offered based on the patient’s commitment to completing the entire series as prescribed by their provider, which allows us to achieve optimal clinical outcomes.

  • Non-Refundable and Non-Transferable: When you commit to and purchase a treatment series, you are securing a block of provider time and resources at a discounted value. As such, payments made for treatment series are non-refundable and are non-transferable to other individuals.
  • Financial Responsibility: By purchasing a treatment series, the patient acknowledges and agrees that they are responsible for the full financial commitment of the package. Incomplete sessions from a purchased series will not be refunded. Discontinuing a treatment series does not release the patient from their financial responsibility for the entire purchased package.
  • Valuation of Rendered Services: If a patient is unable to complete a purchased series for any reason, any treatments that have already been rendered will be valued at their full, non-discounted, individual session price.

Article III: Binding Agreement and Patient Acknowledgment

This policy is publicly posted on our official company website and is accessible to all patients prior to booking. The following terms constitute a binding agreement.

  • Agreement by Performance: By scheduling an appointment and tendering a deposit, the patient acknowledges that they have had the opportunity to review these terms and agrees to be fully bound by them. The act of paying a deposit serves as the patient’s express consent to this entire policy.
  • Policy Supersedes Incomplete Paperwork: This agreement is binding irrespective of whether the patient has completed or signed their formal intake paperwork or other consent forms. While we require all patients to complete their official file, this publicly posted policy serves as full disclosure of our terms. The financial commitment is made at the time of booking and is not contingent upon the subsequent completion of administrative forms.
